Fill out a Shoe Repair Job Form for each pair of shoes you're sending, and print it out. You can choose your own repairs, or choose to let us decide what is needed to be repaired -if you don't know. If you choose your own repairs, please view our Shoe Repair section to review your repairs. Tip:

rubber heels, and also rubber heels with half rubber soles & a free shine. |

Put the completed Shoe Repair Job Form inside the appropriate shoes (1 Shoe Repair Form for each pair of shoes). We recommend you use masking tape to keep the paper inside the shoe. This will tell us what shoes need what repairs (assuming you send us lots of shoes). |
